This key role will provide administrative support the case management nursing staff and collaborate with other members of the health care team to facilitate quality, cost-effective management of our patients with kidney disease. This position works with scheduling, managing patient data, collecting data and other administrative duties. The position is home based and may include minimal local travel as necessary. Overnight travel may be required once per year to attend team meetings, trainings or other required job activities. Position is designed to have a work schedule of Monday-Friday 8 -5 however; the ability to work overtime with little notice is expected.
